Required documents for AP / class of admisison

Hi,
What should I fill in for Class of Admission on AP form.?
I have used AP though I have an H1 since it was not stamped.

What are the required docs for AP. Here is a list of what I think are required. Any suggestions? . For my spouse should I be sending marriage certificate as well ? Should I attach copy of previous AP?


1. I131
2. Copy of I485 receipt
3. Copy of I-140 approval.
4. 2 immigraiton photos with signature and A # on the back.
5. Check for $110
6. Copy of I-94
6. Copy of Passport
7. Copy of Drivers license

If you have used AP at POE then you must enter
PAROLEE in Class of admission

Folloing docs are required
• 2 photos which fits INS requirement with A# and name written with pencil on back.
• A check $110.00 for I-131 renewal fee made in the name of "U.S Citizenship and Immigration Services "
• I -131 Renewal Application
• A copy of Form I-94 Departure Record (front and back)
• A copy of I-485 Receipt Notice
• A letter explaining the reason to grant you AP.
• A copy of last EAD (front and back)
• A color copy of driver’s license (front and back) or a copy of passport front page showing date of birth and photo.
• Marriage Cetificate for secondary applicant.
 
The AP application requires you submit a document that shows your date of birth and photo, you could send your DL's color photo copy. The EAD copy could be a second confirmation for your age proof.

If you dont have it then dont send it, i guess it must be ok.
