Required Documentation for H1-B revalidation

In the FAQ section of this portal the following documents are suggested for H1-B Visa Stamping.

Notarized or attorney certified copy of your entire passport
including the page containing your H-1 visa.

Degree certificates [original + copy]

Reciprocity visa fee is $75

I have received my H1-B approval notice (I-797) and need to apply for visa re-validation at St Louis.

Can someone please inform me if the above documents are needed for re-validation? The information that I have is for submission of Original Passport and Reciprocity Visa Fee of $50, plus other standard documents found in "Required Documentation for H,L,O and P Visas" section of this site.

1. DS-156 form for yourself and your wife if u have.
2. DS-157 form for yourself.
3. One photo for each of us.
4. Orginal passports of both.
5. Fees $100, $50 for each by DD. Total ($300). if you are inDIAN
6. Employment letter.
7. Orginal H1B and H4 petition. And 194's.
8. Self addressed Fedex bill for return of passports.
