Renewing expired green card.

I have been a green card holder for 36yrs and just found out that my greencard is no longer valid due to it being old and not having expiration date on it. I also have noticed that there is an error in my middle name on the card...and I have remarried so I have 3 questions that INS wont answer for me. ....I need to fill in a I-90 but my problem is in that it ask to check ONE box regarding why Im filling it in...1. renewing expired card 2. changing name. 3. changing a error on card. INS wont tell me what box to check...and I dont know what one to check cause all apply. If I check renewing expired card they will question my name change...therefore not except my form. If I check name change due to error they wont axcept due to new name.... and then if I check change name then will the kick it due to it being expired.....lolol
This is so complicated and they wont tell me what box to click....and why do they have a phone line...?? Please someone give me some insight to what I should mark.....??

Green cards without expiration dates are still valid. There was a proposal to invalidate them, but it's still just a proposal. So for you, renewing expired card is not applicable.

Pick the name change option. When they print the new card with your new name, that will fix the old error with your name.
