Renewal of EAD

I have to renew EAD and AP. Could you please help me with the following question.

1. TO what name should I write the check to, (INS, BCIS, etc).

2. How many photos should I send for EAD and AP.

1. Write your check to BCIS.
2. Two for each.

You should do it on line. There are plenty of explanation over there. If you still want do it through snal mail go to bcis.gov and fine form I-765. It contain the explanation what and how to do it.
