Renewal EAD

I would appreciate if somebody answers the following questions:
1) What is the filing fee for EAD renewal ($88 or $120).
2) Among the documents to submit for renewal EAD, I see previous I765 receipt notice. I have I765 approval notice. Does it work?
3). In the I-765 application, there is an entry for previouslyapplied service center and "Date". What date is this? Notice Date, Receipt date, filing date?
4)I see that some people get iEAD before 90 days. Is it possible? Depends on the SC ah?

1> From 27th Feb, The fees are back to $120.
2> It says a copy of last EAD. i don't see it says EAD's receipt notice. I am wondering, if they want the top tear-off portion of EAD's approval or a copy of EAD card ?

5> Who shd we address the check & the application to ? BCIS ?
6> which I-94 card shd we send ? The new H1 i have, I don't work for that company anymore.
6> current immigration status? can I write AOS pending
