regarding filling I 134

I am just looking at form I 134(Affidavit of support). I have the following questions:

1. Do I need to fill one for myself and another for my spouse(I am the primary applicant and my spouse is the dependent)

2. Who is the sponsor- is it the primary applicant or is it his employer?

3. whose name do I fill in item no 3 where it says"This affidavit is executed on behalf of the following person"

4. what do I fill out in item 11 about specific contributions to support- should I check I intend or I do not intend? and should I mention regarding the contributions?

This form looks complicated. Gurus please help.

Hi , as i am browsing this forum for the very same reason . My attorney gave me I 134 to fill but i heard its only for visitor visa sponsor not for Employment based immigration . so multiple sources told me its actually i 864 which is more complicated than I 134 .
So , pls verify before filing

Hi,
I am in a similar situation where my lawyer has asked me to fill I-134..none of my other friends who are applying are filling this form.. Is this really required or is it more of a "nice to have"
