Reg. Mothers Non Availability Birth certificate...

Hi

I am trying to get Non Availability Birth Certificate for my mother. In the non-availability certificate what last name should I use her maiden name or married name.

There are two places I have to use my mothers name:-

1. has been made on the request of XXXXXXXX

What last name should I use here.

2. Found that the event relating to the birth of XXXX daughter of ____ was not registered.

What last name should I use here.

I believe the NABC should be on the maiden name. It relates to the birth and hence should be on the maiden name. The relationship between the maiden name and the married name is established by the marriage certificate.
