1. Copies of both sides of your form I-94 Arrival/Entry Document;
2. Copies of Both sides of your form I-20A-B/I-D(F-1 students and dependent);
3. Copies of any and all Employment Authorization documents;

I have a question, there is a I-94 portion at the bottom of H-1b approval notice, would that make list 1 and 3 the same?

"On January 7, 2005, we mailed a notice requesting additional evidence or information in this case. The notice explains in detail what additional material is needed. If you have questions about the notice, what is required, or if 14 days have passed and you have not received the notice, please call the National Customer Service Center at (800) 375-5283"

What kinds of requested additional evidence should be expected?

RD=ND=6/29/2002 (EB2 or EB1)

Why would they ask for I-20A-B ? Did you not travel outside US after graduating from the grad school here?
