Reentry Permit Help

I'm GC holder. Want to apply for reentry permit.

1).What is the form for Reentry permit.
2).Should I apply through Lawyer or Can I apply by my self.
3).What is the best reason that I can give (actually I’m going for an employment, which I cant mention as a reason).

4).I’m planning to leave US in March 2004 for 2 yrs.
If apply reentry permit in December ,2003 probably I will get it in end of 2004.
So can I give intended dates of departure and arrival as (03/2004 and 03/2006).
Probably I may need to visit US once( to avoid expiring GC after 1 yr absence ). That is ok for me.
