Reasons why "RFEs" are generated

If you file your case through an experienced lawyer firm like mine (FRAGOMEN) the number of RFE generated tend to be less - from a discussion in my office cafeteria.
The apparent reason is that they file all necessary documents + some extra. Somehow I feel this is true because at my client site, in the recent past there have been 20-30 approvals and I haven’t heard any RFEs. Any thoughts?
 
Feeling good about your lawyer definitly gives you some

peace of mind, which is important at this uncertain time...If he(she)\'s really that good, then , you\'re just one lucky guy.
