Please help me with few questions regarding REP. I have received my GC last year. This is the first time I am applying for Travel Document.

1- Part 1 Question 3 - Class Of Action
What should I write?

2- Part 3 Question 1 & 2 - Date of Departure, expected length of trip
Can I skip this question since I don't have any plan at this time?

3- Part 4 Information about your proposed travel (Purpose of trip & list the
countries you intend to visit
Do I have to write name of all the countries that I may visit? What if I miss the name of the country and than I visit, would that be a problem?

4- Part 5 How much total time have you spent outside of US

Should I choose option 1(less than six months), or write none, since I never been outside of us in last 15 years?

Date of Departure, length of trip: Put a date, it doesn't matter, length just estimate
Countries: Put countries you may want to visit.
Time outside the country: less than 6 months.

Anyone has answer for question 1, and I am not clear with answer for question 4. Just in case if I dont put contry name when I am appying for REP and in the future if I visit that country (I am not concidering to visit COP) would that be a problem? I know anthing is possible with USCIS.

List of countries is just a list for informational purposes. You are free to go anywhere including your COP even if you dont list the countries. I doubt they will ever make a big deal if you wrote Germany & went to Germany & France.
