Re-entry Permit -- Queries Gurus Help needed

Gurus,

I got my GC a year back and now I am planning to go back for a year or so to take care of some personal issues.

#) Do I need a lawyer to apply for a re-entry permit
#) Where can I find the application form?
#) I want to take up an assignment in india for an year through my parent company. How do I put this in the application form? Is this accepted by USCIS?

Do let me know if i have to take care of any other stuff as well
