re entry permit help needed

Hello,

i'm about to file a reentry permit and I need some help to understand the following:

*I am planning to leave after filing and to come back for the biometrics, should I put the date of actual leave or a fake one 3, 4 weeks after filing?
*the inputable form online only give 2 digits for the length of trip, and it says days, I think this is a bug anyone experiencing the same?
* is form date 3/22/13 the latest available?
* I'm a LPR green card holder, what should I put under class of admission part 1, pt. 6. I do not seem to find a suitable one for LPR
* how do I tell them that I want also the notice for biometric sent to the lawyer?
* should I add a cover letter? (maybe mentioning the pt above and why I want to the permit)
* what docs should I add to the application?

1. it takes 3 to 4 weeks to get a fingerpring notice, what is important is the mailing date.

Write the date you plan to leave after biometrics.

Print the form and write it in by hand.

Check the USCIS web site on their I-131 page.

Include form G-28 with your lawyer's details. Don't choose your lawyer's address as the mailing address. With the G-28, they'll send the biometrics notice both to your address and your lawyer.

on Part 5 it say: Total time spent outside US since getting the GC.

my situation is this: since getting the card +/-10 month, but since my last visit 2 months. Do I risk a denial if I check the box 6 to 1 yr. (the truth), or better take the risk and check the box "less than 6 months"?

I guess if one download the form directly from their site, it must be the current one.

Include form G-28 with your lawyer's details. Don't choose your lawyer's address as the mailing address.

Clarification: I meant don't write your lawyer's address as the mailing address for your I-131. But on the G-28 you'll put all the lawyer's details as requested.

You risk denial if you lie.

Generally the amount of time outside the US won't affect your Reentry permit approval unless it's more than 4 years.

I just realized that on my passport stamp they put RCA but not the alien #, apparently this is just a courtesy and the Immig. officer does not need to put (or at least some do not).

My problem is that on the application they ask for the A #... what should I put RCA or say that they did not put it?

If you don't know your A#, you'll just have to leave it blank. They'll look it up from the other information you provide (although this may delay the processing a little bit).
