Did you change address when you changed jobs. Did you notify the service center about job changes.
Mine is very similar to yours although ND is many months later.

Did not inform about change of jobs.
Applied for renewal of work permit at Dallas address. Received the same at Dallas address.
Applied for renewal of AP at Dallas local INS office. Received the same.
