I don't have birth cert so initially i submitted one affidavit and non-availability cert from local municipality. INS wanted one more affidavit.
I got a billing notice in Dec 01 saying that fp fee was not submitted and had to reply to that by sending copy of cashed check, that is the reason spouse has early fp date
Other minor details no job change. I had 2 promotions from my intial LC date (4/2000) and salary was 15% higher. Did not take tetanus shot due to shortage, doctor gave waiver. Spouse used married last name in 485 app although the passport had her maiden name, we got her passport changed after we filed for 485
2 EAD's 2 AP's and applied for 3rd ap in mid-may.
