In my mom's RIE which was recieved on Jul 25, they asked for three thing:
1. Form I-693
2. SUpplemental Form to I-693
3. G-325A.
Three forms in total?!Thats more than what I expected. Can anyone share their experience and tell me what their RIE were asking for? Replies were greatly appreciated!

I-693 and Supplemental Form to I-693 will be completed by a doctor on the lit of "Designated Civil Surgeons", available on the BCIS site. You can find one on this site http://uscis.gov/graphics/exec/cs/index.asp

The G-325A is a form that you (or your lawyer) need to fill out. It is basically asking for the places you lived/worked/went to school during the time from the initial application filing until now. If you have a copy of the initial G325A that you filled with I-485, all you have to do is use the information from the initial G325 and add anything that has changed (new job, new address etc.)
