Questions Relating to RFE on I-458

I have recieved a notice of action I-797 form which asks for the following

- photocopy of all pages of passport
- additional evidence of birth such as early school records, medical
records, religious records and/or two affidavits
- letter of employment
- earning statements for two months
- federal income tax returns for last year.

I would appreciate if any of you could help with following questions:

1. Is there a format for the affidavit? Who can provide the affidavit. I
do not have a birth certificate and had provided a non-availability
certificate from the municipal coroporation and an affidavit from my father
(my mother is deceased).

2. The letter of employment asks for prospects of continued employment.
What should the letter say - I am in a tenure track faculty position but
there is no guarantee of tenure.

3. My wife also got a similar letter. Her letter has an affixed barcode
while mine does not - should I worry about that?

Affidavit can be provided by anyone who was present when you were born (not necessarily present in hospital/place of birth, but who "knows" that you were both that day). You relatives - uncles, aunts are normally good options. But if they are not available your parents friend can provide affidavits too. If your elder siblings/cousins were adult enough (when you were born) , they can provide too.

2. employment letter should mention these three info
- employment date
- your job title
- your salary
I don't think it should mention anything else.

3. don't worry about barcode.
