Questions regarding the documents need for stamping...Urgent

Folks:

  I just got my approval notice (courtesy copy) from INS today. It says that I need to bring the following:

-1. this notice
-2. arrival/departure record
-3. I-94
-4. Passport
-5. two photos

  My question is regarding the second (2). What is arrival/departure record ? Is my old passport (along with my current passport) O.K. since it showed all the entry/exit time stamps ? Or I have to type a piece of document listed all the entry/exit time/location since the year I came to the states ? Your help is appreciated.

Thanks, GPV96. But it listed arrival/departure record and I-94 are two separate document INS mentioned ? We only keep the latest I-94 on our current passport, right ? Any ideas ? Thanks a lot.

All my previous I-94 were taken back when we travelled. ?

we only have our latest I-94 in the passport with us. Does I-94 means arrival/departure record ? I thought they are different things since on my approval notice, INS asked us separately to bring both I-94 and arrival/departure rocord with us ? Does that sound right ?

i just went today for PP stamping. They took the I94 white form which was attached to the passport. I had taken other I94 , like the one for H1 extension after the arrival to US, but they did not ask for any and since they did not ask I did not give.
