Questions regarding DS 156...

Hi,
I was on F1 status but now I am on H1B visa. I have been working on H1B from past 4 months.
I have an VISA stamping appointment at Vancouver, Canada on 9th of March. I was filling DS 156 form and I have following doubts:

1) Name and Telephone Numbers of Person in U.S. Who You Will Be Staying With or Visiting for Tourism or Business ?
Now I am already staying in US in an apartmenm so what kind of answer do I give here


2) How Long Do You Intend To Stay in The U.S.?
Now I have H1B till 2008. So should I write till 2008 ?

3) What is purpose of your trip ?Now what kind of question is this ? I am working here on H1B so what do I write here.

4)Who Will Pay For Your Trip?

Is it ok to say myself.

5) Have You Ever Been in The U.S.?
Yes No WHEN?
(Most Recent) day month year
FOR HOW LONG?

Now I am completely perplexed at this question. What do I write here. I came to united states as a student in dec 2002. Then went back to India once and then again came and then again here from 4 months.
So what do I write here ?

till your h1 expires. i.e 2008
