Questions on filling forms - 485/EAD??

I am filling the forms for me and my wife and am confused with a few filelds - could some of u experienced guys help me out please:

1. A#
a. What is the A# - 140 application number?
b. Is it only applicable to me or my wife as well? (dependent filing)

2. Visa #: Is this the control number on H1 visa or the LIN number of the H1 visa?

3. BIOGRAPHIC INFO - Wife's last name - should this be the maiden last name of the after marriage (new passport) last name? The form is kinda confusing on this one..

4. I-765 (EAD) - Section 11 asks if "u have every applied for EAD before" Should I include the EADs applied for as a student as well?

5. I-765 (EAD) - Section 16: Is the Eligibility criteria for both me and my wife is "c-9"??

6. I-131 (AP) - Section 7: Does someone has a sample letter explaining the "how u qualify for AP" as asked in the section by INS

7. I-485 - Part 2: Is application type (a) for the primary applicant and (b) for spouse?? or is it (a) for both??

Any questions u can answer would be a great help!

1. A# - alien number. If you have never applied for I-485, asylum etc etc and never been assigned one, just put "None". In most cases you don;t have one when you follow a straight-forward route for GC. They will issue you A# number when the application is filed - it will be on your receipt.
2. H1-B visa number, not approval number. Basically the one printed in teh lower right-hand corner in red.
3. If you are referring to G-325A, doesn't it say "for wife, give a maiden name"? ( http://www.ins.gov/graphics/formsfee/forms/files/g-325a.pdf )
4. Not 100% sure, but looks like it doesn't matter why or when you applied before - so I guess all cases in the past count. Might want to double-check on that though.
5. Not sure. Guessing (c)(9) for both of you because INS defines (c)(9) as "I-485 pending" ( http://www.ins.gov/graphics/formsfee/forms/i-765.htm )
6. I think all you need is either I-485 receipt ( which you don't have yet :=) ) or a copy of an approved I-140. This was enough for me. No additional documents were enclosed and AP got approved.
7. I think it's (a) for you and (b) for your wife. Not sure though.
