Questions on filing EB2-PERM

I am having BS plus 8 years experience,but no MS.
But my salary levels should be well above prevailing wage etc.
Questions:-
1.So,would I be able to file under EB2-PERM without issues??
Also BS + 5 plus years with salary above prevailing wage..will it
qualify under perm,eventhough no MS???
2.Which job code/job title etc should one choose so that the chances
of getting approved are more??

3.Also,if you are employed in a different state and Employer is headquartered in a different state,how does it work-is it the place where you are employed determines job code or place where Employer is HQed??

It depends. What does the job ad say? If it specifically says it requires the MINIMUM of a master's degree in such and such field.." then, you should qualify. The wage is important, but not the sufficient factor to determine EB category..

You should choose the job title that most closely describes your actual job.

The geographical location is irrelevant, I would think. An example: a high school math teacher in CA and a high school math teacher in NY would be choosing the same job code..
