I am filling my DS-156 form and i have following questions.

1) Column 25: Name and Telephone Numbers of Person in US......
I am on H1 and planning to get stamped in chennai. what should i write here?

2) Column 29: Have you Ever Been in the U.S.?
When?........for How long?..........
I made two visits so far. So should i mention my latest visit or first visit?
