Questions on AC 140 paperwork

I have a couple of questions on the Ac 140 paperwork

1) The lawyer has sent DS 230 with the application to Mumbai, do I need to fill out another form or just take the copy of the first one to the interview?
2) On the affidavit of support I 134 question 1d what do I fill in for "A" number.

1. Take an extra DS 230 Part 1 and 2 to consualte. It is also avaible in P4 documents.

2. Leave the A number blank, if not available.
