Questions for I-765 for iEAD

Can you pl. tell me what to fill in these fields of the I765 form when applying for iEAD through InfoPass?

10. A-number: Do I fill in the A# from the I-485 receipt or fill in I-94 number?

11. Shall I say YES? If so, then what shall I put in for "Which USCIS office did you apply to?" and "Results"?

15. What do I put in for current immigration status: I am on H1B but applied for 485/130 after marriage to USC.

16. Will this be (c) (9)
