questions about I-765

Hey,
I'd like to renew my EAD. I have two questions about I-765.
1. Eligibility categories for Item 16. In my case(EAD renewal), is c9 the right answer?
2. Is it mandatory that we need to send Bule Signature Card along with I-765 (EAD) application. If so, may I download it from internet? Where?

Thanks a lot. Really appreciate your help.
 
1. it is C9 if you have I485 pending
2. No signature card any more
