Questions about H1B1 extension

1. When I first applied H1B1 around 3 years ago, there are no such fees like: training and fraud. Now do I have to pay them for the extension?

2. Regarding the question in form I129: "have you ever filed an immigrant petition for any person in this petition?" I have to choose yes, since my I140 has been approved. How should I explain it? What should I attach?
