Hi ... This is for a friend. Please help with following questions about for DS157:

Column 12: List Your Last Two Employers
Ques 1: The Last two companies have changed there names and have also relocated. Should he give old names/addresses OR the new ones.

Column 17: List All Educational Institutions Attended ... But Not Elementary Schools.
Ques 2: What is INS definition of "Elementary" - is it Class 1 thru 8?
 
DS 157

(1) I would put old names and put new names in brackets. I would rely on what was employer name on my old h1b as the basis of info ( name and address).
(2) As I undersatnd elementary means upto 12 years of education. So I had put my bachelors degree and the institute.
