Questions about DS 230

I want to ask following questions about DS-230 form.

- On Item #44, Since that my daughter is just 7 years old and I will have to fill this form on behalf of her, should I write my name there and check ‘yes’?

- Since that my daughter is just 7 years old, she can’t sign that form by herself... maybe she just can write her name there. What should I do ? If I have to sign it, should I write my name on paranthesis or something like that ?

- On Item #31a, Should I write only my wife's and daughter's name or should I write full name there as Family Name, First Name

I think this part explains itself. If you are the one completing the form on your daughter's behalf, you have to check YES, and put your full name and your relationship to your daughter. (Yes, Your Full Name, Father)

Each applicant must personally sign his/her own forms. However, if a child is not capable of signing the forms themselves due to age limitations, either parent may sign the form on their behalf.
