Questions About Affidavit I-134

Hi All.

Im about to ask my brother in law to file an affidavit I-134 on my behalf, altough I have some questions about the things that need to be atached to the I-134, I've compiled some info and here is a summary of things needed.

- Past 3 years of income tax returns, along with their W2. (tax transcripts available free from the IRS or copy of your 1040).
- Letter from their bank indicating how long you had your bank account, how much the average balance (if you tell the bank they want this letter for immigration, the bankers should know what to write), It is preferably that this letter is signed by a bank staff.
- Employment and Salary Verification (Letter from the Employer).
- Copy of your immigration papers [GC/American passport].


Also the obvious the form needs to be oath and notarised in a notary I guess that most banks have one on site, so here is where the obvious question arise, do any of the xerox copies of the documents included need to be legalised/notarised also.

Second the latest form I've downloaded from the USCIS says on top of the page

Is this the latest form, and will it be acceptable? altough it says that xpired on 04/07.

Before you click on the "Download I-134" it says "Edition Date:
2/28/07. Versions 12/04/06, 7/14/06 and 6/17/06 are also acceptable." So, not only that but even the older versions are available.
 
A. Statement from an officer of the bank or other financial institutions where you have deposits, identifying the following details regarding your account:

Date account opened;Total amount deposited for the past year;Present balance.
If sponsor works for somebody or has a job then-
Statement of your employer on business stationery,revealing:
Date and nature of employment;Salary paid;Whether the position is temporary or permanent.

If your Sponsor is self employed THEN only he/she needs to submit Copy of last income tax return filed; or Report of commercial rating concern. (But if you or your sponsor wants to provide TAX doc's then you can but that is not necessary)

Copy of sponsor's immigration papers [GC/American passport]. NOT required (again if you want to add then ..)

BUT I-134 must be notarized
