Question when to do medical and other docs preparation

I am waiting for Sept 2014 visa bulletin to figure out what my situation will be. When do people do their medical, after being confirmed being current in the visa bulletin and booking the interview date at the consulate?

Also to do your medical do you need to wait to get an appointment with the approved medical doctor by US Immigration?
 
If the VB gets readjusted and includes your case, you will receive a 2nl within a week later, and you can just take it with you to the panel doctors asigned by the embassy, and do already your medicals exams.
