Hi eltoro,

When ur doctor forgot to tick the checkbox next to the TB test. what did u do to remedify this error? I am almost in the same situation now. the doctor seems did not send my immunization record and my wife's TB test results.

Thanks,

I went to the same doctor who did the original health check (fortunately he was still in business after these two years) and showed him the RFE. He said it is just a clerical error. He made a xerox copy of the form, ticked the TB "no reaction" box, initialed the change, and signed and dated (with the current date) at the bottom of the form. He also wrote a sentence explaining that it was just a clerical error and the results are kept on file since 2002. He also made a copy of the immunization sheet, put that in the envelope, sealed it and stamped the seals with his "sealed for immigration" stamp. It obviously worked for me, as I got approval two business days after they received the response.

Hope this helps.

Thanks
