Question on Wife's Affidavit

Hi Folks,

I have a question on my wife's affidavit. My wife has a passport before the marriage with her last name. I got married and changed her last name in passport with my last name.

Inorder to apply for GC she doesn't have date of birth certificate. Now inorder to get the non availability certificate and affidavits which last name I have to use.

What is the last name it has to be in Non availability certificate and affidavits.

And one more question is how many affidavits do I need and with whom !!!

you can always get her birth certificate and affidavits with her maiden name on it. But, make sure you keep some kind of document which prove the "last name change".
