question on the 'basics' - CSC

dear fellows
I am new to the concurrent filing issue, so I am hoping to get some info and confirmations from the experienced filers.

My i-140 (RD 6-22-2004) was filed at CSC with consular processing option and is not approved yet. Now I would like to submit I-485 because I want to switch employers soon.

My questions:

(1) What documents are absolutely necessary for the I-485 filing?

I-485
G-325A (Biographic Information)
Copy of I-140 receipt (my employer did not give me a copy - can I say "please link application to WAC-----?)
I-693
I-94
Birth certificate
copy of nonimmigrant visa in passport
2 photos (USCIS style)
Employment letter
Medical examination (I read in another post that this can be submitted later and, if not received beforehand, will trigger RFE)


These appear to be optional:
G-28
I-765 EAD
I-131 Advance Parole

(2) Can dependents file separate I-485s four months later than principal applicant?

(3) How does one get notified about fingerprint appointment (by CSC) -- through email AND letter?
