The SC form asks for father's name and immediately below it is a field for date of birth. Are they asking for the applicant's date of birth or the applicant's father's date of birth? (considering that they already asked for the applicant's date of birth at the beginning of the form).

The SC form asks for father's name and immediately below it is a field for date of birth. Are they asking for the applicant's date of birth or the applicant's father's date of birth? (considering that they already asked for the applicant's date of birth at the beginning of the form).
It is supposed to be father's DOB, incase you don't have it, fill with your dob or some dob(they really don't care - I believe)
I do i send the $175 appln fees and $20 mailing fees in separate money orders/ cashiers checks?
Choice is yours, you can also combine it togeather.
