Question on Medical RFE

Today I got the rfe on my medicals, stating that the doctor missed some information. my last medicals were done in april 2004. My question is do I have to go to the same doctor or I can choose a diff doctor.

Any one who got the same RFE - From VSC

RFE:
Your Original form I-693 is being returned to you at this time. You are required to return to the physician who completed your medical examination to obtain a completed supplemental form to I-693 to establish that you are current for all recommended age-specific immunizations or that you have received a waiver of that requirement in accordance with section 212(g) of the act. After the supplemental form and the I-693 have been completed and signed by your physician, you must return both forms to this service. Please note that both forms must be returned to this service in an Unopened envelope that has been sealed by your Physician.
 
In my case, it was clearly written to go back to same doctor. But, I was not asked to send both forms. Just the supplemental.
 
neopost07 was also asked to go back to the same physician who did his initial I-693. To keep things simple it will be better to go back to the same doctor. I would guess if you go to a new doctor now, you might have to repeat all tests (TB, immunization tec) again and file a fresh I-693.

Thanks for your input.

In my case my original I-693 form was sent back to me, it has my old address and the tests were done in 04/2004, it is almost three years. The old form is OUtdated, there is a new version on the USCIS site.

Do I have to refill a new I-693 form with old information or just send back the OLD form as it is plus the updated supplemental form.

It is better to use what you have. In fact, you don't need to do anything. Your doctor should be able to take care of that.
