Question on filling FOIA form

Hi,

I am trying to fill out the FOIA (g-639) form. I need the documents for filing ac-21. (Eb-3/VSC, 140 approved and 485 > 180 days)

1. Should i request this for my spouse too (spouse not working)

2.if so, should i use different application. Do i need any docs related to her (I have photocopies of 485 recpt for my wife)

3. On the form Item no 5 - What should i write ? (they wanted name,date , subject matter,location of Information)

4. Also what is the Purpose - (can i say file for AC21)

5.On item No:6 on the form -Petition or claim receipt No - what should i write here?

can any one clarify these ? Need to request this soon.

---> No need - you already should have all her docs. If u still want to feel good & safe, go ahead and ask for her docs as well as it doesnt costs anything.

---> Yes, u need a different application. They get confused if you include multiple cases on one application.

----> Whatever info u have. U cannot have location info.

----> You do not need to mention AC-21. Just say "for my personal records"

----> Ur 485 case number

You would mainly need the following:

1. ETA 750 A&B (Labor documents) which specify your job description, requirements, salary and title. If you don't have these, you can get them by filing FOIA as explained above. Your new job should be similar to the one in ETA 750s.

2. Job desc letter from new employer
