Question on EAD application

Guys,
I need to apply for 3rd EAD ... under "Have you ever before applied for EAD" what should I enter for "If Yes, Date(s)" field .. I mean should I enter date for just the last EAD or both 1st & 2nd EAD's?

Enter "yes" and enter date for your last EAD. That is what I did, and I was approved last Thursday.
