Question on DMV record

I just received my DMV record.

I have 2 items there.

First is an accident, it has a docket, citation or fd file number but no point lost. I was never cited for it.

The second one is a speeding violation with no docket, citation or fr file number, just one point lost.

Question: Should I report these 2 items? only one of them or none of them, the speeding violation not having a ref. #?

Also I paid the ticket for the violation. For the accident there was nothing, not even a police officer at the scene. I guess that the insurance reported it to the DMV.

The question I believe on the N-400 form is whether you were ever cited, arrested or detained ....

so use the above criteria as to what to mention and what to not.

If you got a ticket, I assume you were cited in which case you will need to mention that.

In case of the accident unless you were arrested or detained (probably not based on the information above), you don't need to mention anything.

More, regardless of what shows up in your DMV history, you need to write down about all cases where in you were cited, arrested and/or detained..

For e.g. in my case, I had a minor accident and 1 speeding ticket, none
of which shows up in my DMV record history. Rgdless, I did mention about
my speeding ticket on the N-400 form. it doesn't do any harm.
 
Thanks. My speeding ticket is reported with no citation info in the docket, citation or fr file number, so was I really cited with no citation ref. number?

How should a citation number look like?
