Do I have to submit a copy of J-1 waiver approval along with I-485 application or proof of H-1 visa for 3 yrs is sufficient to show that one has served 3 yrs in an underserved area. I am applying for I-485 after 3yrs in underserved area based on labor certification.

I am not sure as I did CP and not adjustment of status. In my opinion you should submit a copy of your J-1 waiver approval as well as proof that you have worked 3 years in an underserved area ( e.g. copy of W-2 forms, copy of H-1B approval, letter from employer confirming your start date and the fact that you have worked 3 years full time in an underserved area)
