I understand that there is a section on the application that a physican has to sign but when Ins services did not request any medical examination they just requested for me to complete part C of the I-485 application and reason why i used a fradulent passport, and i completed both parts and now i want to know do i have to have a physican sign the second part of the application if they didnt request it. Another thing that im concerned about is that Ins services requested me to explain why i used a fake passport to enter this country but i entered this country with my real passport do i list this in my application or do i call Ins services to see what they can do

Just do waht they ask for. IF you never came on afake passport..did ucome on a fake visa? Anything fake?

no, i didn't come with afake passport .

Is your I-602 related to medical? if yes,Doc must fill it out.If no,leave that part blank.
You entered legally the US,make a copy of your visa in your passport and mail it out.If you still have your I-94,also send a copy.You're not gonna fix the situation by making a phone call.
