Question for Ds-156 and DS-157

Hi,

I am planning to go H1-B stamping to ottawa, canada. I have completed MS in USA.

I have few questions in DS-156, can you please tell me what i have paln to fill is correct ?

Q15. Home Address: Current USA Address or India Address

Q21.Present OCCupation : Programmer

Q22 When you intend to Arrive in the US: Return date from Canada ?.

Q26.How long you intend to stay in US: Validity of H1-B (which is stated in I-797)

Q27. What is the purpose of your trip : To work .

Q41. Date: Since we fill the Ds-156, before the actual interview date, should i date on which my interview is scheduled or the date when i submit this form ?

and in DS-157

Q8: Full Name and Address or Organization in the United States:
Employer Information/Address ?

In DS-156

DS-157:
Q8. Full Name and Address of Contact Person or Organization in USA: Current Empkoyer Address ?

Q18. Have you made specific travel arrangements ? : No
(I guess they want this info only if your on B1 or Visitor Visa, becuase are aassking compelte itinerary ?)


There is another form DS-158 i think which is not required for H1-B Correct ?
