Question About Renewing Green Card

I received my green card through the amnesty program in the 1980s and I have to renew it. I’m trying to figure out the question in part 3 of the I-90 form. It asked if you entered the U.S. with an Immigrant Visa, also complete the following:

Destination in U.S. at the time of Admission

Port of Entry where Admitted to U.S.

I came when I was 3 years old on a visitation visa (not immigrant visa) and stayed. Do I put the information from that passport on the application or do I simply put N/A?

You did not enter with an immigrant visa, and the question appears to need answering only if you did. If that is the case, then you would leave that answer blank as it does not apply to your situation.
