Question about H1-B extensions

I know that if I filed my PERM before my 5th year of H1b, I can then extend my H1b in 1 year increments.

BUT what if i filed my PERM 1 year before my H1B expiration, and got it approved 3 months later. At that time, my H1b will only have 9 months left, which is not enough time to wait for my I-140 to be approved, and I can't apply for 1-year extensions because it's less than 365 days. So what can I do in this case?

Some say that H4 and H1b time count together to a total of 6 years. Others say they do not count together. Is there a definitive answer somewhere?

My situation is that I moved from H4 to F1 then to H1b. While I was in H4, the H1b holder did the 1-year extensions. Does that mean that I am not no longer eligible for such extensions when I get into H1b? If H4 and H1b do not count together, then I'll only have fewer than 4 years of H1 left.

What are the exact rules regarding this?

How do I "qualify myself" for such 7th+ year extensions?
