Question about EAD (Paper Filing)

I want to file my second EAD in local INS office.

Question: When my lawyer filed first time, he had almost blank paper with my signature on it with the I-765 form. And when I downloaded I-765 from BCIS.gov that page is not included in that.

Do we have to include that signed page? and if yes, where can I find that?

I don't remember signing one. Since you had a lawyer, I wonder if they wanted your signature for the G28 Form, which informs BCIS that you are represented by an attorney.
