Question about AP

I am about to file my third AP.
My previous was filled by company lawyer, but now he has increased his fees, so I am thinking about do it by myself.

I got couple of questions,

1. What should we fill in "Class of Admission ?"

2. Should we write on separate paper our eligibility criteria or attaching copy of 485 receipt is sufficient?

My last entry in US was on H-1B.

Answer for Q2.
I never filled eligibility criteria, left blank. Not to forget to enclose a copy of I-140 approval.
Enclosures:
1.copy of First page passport, showing your photo
2. 485-receipt copy, copy of I-140
3. copy m.c. for spouse
4. copy of B.C. for children, ofcourse fee and photos.
