PWA form question

The employers representative thats handling my PERM case wrote my name as person requesting the form followed by my address. I already received the PWA from State Dept of Labor and actually it was maled to my address.

Should she have put her own name as the person requesting the wage determination. SHould I have her reapply with corrections.
