Process to request AP [pending for 5 months] expediate from VSC

Everyone,
What is the process to request expedite AP. I know a couple of people have sent faxes to VSC. Is there a form or format that I need to use.
Somebody who was successful, plz post their expereince. Thnx.
Mine is pending at VSC since Nov 3. Now I moved to California Bay Area, making it difficult to visit Newark, NJ.

Don't worry. You'll probably get AP in a week or two. Mine took 5 months and two weeks. (ND 10/22/03; AD 3/30/04)

Since my EAD is also pending. I am worried.

My EAD RD is Nov 3. I see people with ND even in Jan, 2004 getting approvals of EAD from VSC.
