Process for applying for PIO card for minor-NYC

Is this a complete list of documents(from earlier post) while applying by MAIL for a minor(US citizen) at NY consulate ?

a. Completed application form, 2 copies
b. Photographs - 4
c. Notarized copy of minor's US passport(first 3 and last 3 pages only).
d. for minor- Notarized copies of parent(s) Indian passport(first 3 and last 3 pages only)
e. for minor - Notarized copy of birth certificate
f. Fees($155)

Can anyone please confirm?
Has anyone applied for PIO card by mail and got it successfully?
What about proof of address?

You have all the documents required listed. I don't think that the NY consulate has any requirement regarding proof of address. Please read the instructions carefully, as they may vary slightly by consulate.

Please remember to add the appropriate amount if applying by mail, so that your child's PIO card can be mailed to you - ($20 for Express Mail or $10 for Priority mail). You can combine all fees into one money order/certified check.

I have applied by mail, twice in the past, and the PIO card arrives in approximately 4 to 5 weeks.
 
how to notarize parents' passports

If the parents have turned their Indian passports to the consulate as they are now US citizens, how can the copies of their passport be notarized?

Indian passports are "CANCELLED" and returned with a notation, when someone turns in their Indian passports, after becoming a citizen of another country.
