Practical Training Denial/Delay

My receipt date for OPT is April 4 2002.I didnt receive it until now.I was told by a customer service representative ,over phone,that my application was denied on Sept 17,2002 and I will recieve an official notice.But I didn't receive until now.But I received a copy of a letter from the customer service representative saying that my application was denied.But there is no information on it that why it had been denied or how can reapply.Meanwhile I went through my file for OPT application in my school and found that there was a check on my I-94 record.It was written that INS told them that I-94 doesn't match with the one they have in records.I have the I-94 card with my passport and I never returned to my country after coming to US.I think the INS offical messed with the I-94 number when he was feeding the info on his system.What should I do now?Was my application really denied?(I didn't receive an official copy of the denial, as well as the automated system and online status show up that my app is pending).I have no strengths to hire an attorney at this moment.Is there is any other way that I can legally come out of this problem.Please advice me.

Here is my take at your situation.

INS is known to make errors at the port of entry. It happened to me once.

One of the options you have is to go to your local INS office with your records and they might be able to correct it. Or you need to go to the INS office which has jurisdiction over your port of entry and they shold be able to correct it too.

Your school should have let you know this when it happened.
