PP Stamping and AC21 docs.

I have an infopass appointment to get my PP stamped. I changed my employer after 180 days (so I have to go the AC21 route). I am sure this question has been asked many times before... but

Could somebody please provide me the lists of documents (or a link to any thread that gives the document list) to be taken for PP stamping and for the AC21 to the local office.
